I want to get this to display a character sheet with picture after you submit it. However, I'm not sure what to write or code beyond this point. Could someone help me please?

Do you have a template for what a character sheet would look like? Asking for an output without having the faintest idea of what an RPG character sheet looks like is beyond me. I know what they are, but that doesn't help me to help you since I don't do any RPG'ing.

As far as I can tell, you are wondering how to code a form. Do you have any programming experience? If not, I suggest you get yourself acquainted with PHP.
